Incidents are already common: 46% report incorrect outputs, 38% sensitive-data leaks, and 17% destructive actions. Yet only 17% have kill switches and 31% automated recovery. Security and governance are major barriers to further adoption. Seventy-three percent are exploring unified platforms for agent monitoring, governance, and recovery, while only 13% say current tools adequately meet those needs.
